How to Request your vote-by-mail ballot:
If you are requesting the ballot to be sent to an address we have on file, click here to complete the request online or call 954-357-7055 to request your ballot over the phone. USPS will not forward a vote-by-mail ballot to a different address.
If you are requesting a vote-by-mail ballot to be mailed to an address we do not currently have on file, you must complete the Statewide Vote-by-Mail Ballot Request Form and return it. We must have the request no later than 5:00 p.m. on the 12th day before an election.

Important Reminders:
Missing/Mismatched Signature: If you forget to sign the envelope or if your signature does not match the record, the Supervisor of Elections will notify you. You will be given the opportunity to "cure" your ballot by submitting a Vote-by-Mail Ballot Cure Affidavit (Form DS-DE 139) along with a copy of your ID. This must be completed by 5:00 p.m. on the second day after the election.
Changing Your Mind: If you receive a mail ballot but decide to vote in person instead, take your mail ballot with you to the polling place. The poll worker will cancel it, and you will be allowed to vote a regular in-person ballot.
Picking Up Ballots: A voter or a designated immediate family member can pick up a ballot at the Supervisor’s office. On Election Day, a ballot can only be picked up if an emergency exists that prevents the voter from going to the polls, requiring an emergency affidavit.
Vote-by-Mail Ballot Return Requirements:
Vote-by-mail ballots must arrive by mail or be deposited in a drop box before 7:00 PM on Election Day. The voter certificate on the return envelope must be signed, or your vote will not count. Note: You cannot drop off a Vote-by-Mail ballot at a polling place on Election Day (except to cancel it and vote in person).
