VOTE-BY-MAIL CERTIFICATE/DUPLICATION OBSERVATION REQUEST
Sections 101.572(2) and 101.5614(4)(a), Fla. Stat.
Instructions: Each physically present candidate, political party official, and political committee official (with a matter on the ballot), or an authorized designee of each (“Eligible Persons”), may review/inspect Vote-by-Mail ballot envelopes, cure affidavits, and other ballot materials prior to opening, canvassing, and tabulation. In addition, Eligible Persons may also be present to observe the duplication of vote-by-mail ballots. Any individual wishing to be identified as an authorized designee of a candidate, political party, or political committee must submit to the Supervisor of Elections a letter signed by the candidate, or an authorized officer of a political party or political committee, as applicable, specifically authorizing the named designee to attend inspections on their behalf.
In addition to providing this document, Eligible Persons observing the duplication of vote-by-mail ballots must, pursuant to F.S. 101.5614(4)(a), sign an affidavit and submit it to the Supervisor of Elections.
